(C-009) Diagnostic Value of Thyroid-function Test, Fasting Lipid Profile and Hematological Parameters in Spontaneous Intracerebral Hemorrhage

Background: Spontaneous intracerebral hemorrhage (sICH) occurs when blood vessels in the brain rupture, causing localized damage and possible neurological problems. The factors influencing the development of sICH are not well understood.

Objectives: Our study aimed at determining independent factors associated with sICH and evaluate the diagnostic value of thyroid-function tests, fasting lipid profiles, and various hematological parameters.

Methods: A retrospective study was conducted in a tertiary care teaching hospital in Southern India, involving patients with sICH as cases and patients without sICH as controls. Important demographic, clinical and laboratory parameters, were collected along with thyroid-function tests, fasting lipid profile and hematological parameters from both cases and control. The association of thyroid-function tests, lipid profile, and hematological parameters with sICH was evaluated using logistic regression, and results were presented as odds ratios. Sensitivity, specificity, PPV, NPV, and AUROC were also calculated to assess the diagnostic value of these parameters in sICH.

Results: Significant independent factors associated with sICH were found to be Hb (OR = 1.657, p=0.003), HDL-C (OR = 1.124, p=0.004), SBP (OR = 1.054, p=0.003), TSH (OR = 0.683, p=0.003) and GCS on admission (OR = 0.395, p=0.008). TSH had a sensitivity of 90% and specificity of 69%, with an AUROC of 0.809. Hb levels had a sensitivity of 75.27% and a specificity of 79.07% with an AUROC of 0.836. HDL-C levels had a sensitivity of 76.36% and a specificity of 71.23% with an AUROC of 0.801, for the diagnosis of sICH.

Conclusions: Our study revealed that elevated Hb, HDL-C levels, and SBP were associated with an increased risk of sICH. Conversely, higher TSH levels and GCS scores on admission were found to have a protective effect. These parameters demonstrated significant diagnostic value, integrating these markers into clinical practice could enhance early detection and improve risk stratification.
